Mentor Application Forms

 

Welcome to the Mentor Application Forms Instructions Page. There are two (2) forms available on this page. One is the Online Mentor Application form which can be filled out on-line. The other is the Off-line Mentor Application Form which should be printed and then filled in.

System Requirements:

  1. Your Internet browser must support 128-bit encryption.
  2. If you are using Internet Explorer (Windows), the minimum version that will work with this site is version 5.0, with service pack 2.
  3. If you are using Netscape, the minimum version that will work with this site is version 6.2.
  4.  You must also have Adobe Acrobat Reader version 6.0 or higher installed on your PC in order to use the online application form. If you do not have Adobe Acrobat Reader version 6.0 or higher, you may download a copy from Adobe's web site.
  5.  You should use either an ink-jet or laser printer to print the completed application form.

Instructions for completing the Online Mentor Application form:

  1. Enter the information requested into the appropriate spaces on the form.
  2. Please answer all questions. If you do not do this your form cannot be submitted online.
  3. Your answers must be in English and must use English characters.
  4. Review the information you entered for accuracy.
  5. Press the "Submit by e-mail" button at the end of the form. This will cause the default e-mail service to open. The form will be sent as an attachment from your e-mail so remember to click "send".
  6. You may print the completed form if you wish. Simply select the "print" button which is beside the "submit by e-mail" button. Please view the "How to submit the Mentor Application Form" document.

Instructions for completing the Off-line Mentor Application form:

  1. Enter the information requested into the appropriate spaces on the form.
  2. Please answer all questions.
  3. Your answers must be in English.
  4. Review the information you entered for accuracy.
  5. Attach a passport size photo of yourself with your completed form.
  6. You may fax the form to Youth Opportunities Unlimited at 759-2081 or you can mail or drop the form off at 4 1/2 Camp Road, Kingston 4.
